  • Early Identification & Employee Wellness

Spot Early Warning Signs of Stress or Substance Use

EAP professionals can prevent escalation by recognizing early behavioral and performance indicators. This guide helps identify subtle warning signs while maintaining trust and confidentiality.

Workplace Wellness Questionnaire

Promote a healthier, more productive workplace with our Workplace Wellness Questionnaire, designed for employees to self-assess stress, engagement, and overall well-being. This confidential tool helps EAP professionals and supervisors identify early signs of burnout, behavioral health challenges, or substance use concerns. By using this questionnaire, organizations can encourage proactive wellness, support early intervention, and foster a recovery-friendly workplace culture.

Behavior & Performance Tracker

Effectively monitor employee behavior and performance with our Behavior & Performance Tracker, designed for EAP professionals and supervisors. This confidential, structured tool allows EAP teams to document observations objectively, track patterns over time, and support early intervention for workplace challenges. By using this tracker, organizations can identify potential stress, behavioral health concerns, or performance issues, promote timely support, and maintain a recovery-friendly, compliant workplace culture.

Early Engagement Strategies for EAPs

Support employees proactively with our Early Engagement Strategies for EAPs, designed to help professionals identify potential wellness, stress, or behavioral health challenges before they escalate. This guide provides actionable approaches, communication techniques, and intervention strategies that foster trust, encourage help-seeking, and strengthen workplace well-being. By implementing these strategies, EAP teams can promote early intervention, improve employee engagement, and create a recovery-friendly workplace culture.

  • Screening Questionnaires for Substance Use Awareness

Note for EAP Professionals: These questionnaires are screening tools only. They cannot diagnose substance use disorders, but they can highlight potential concerns and encourage employees to seek professional help.

CAGE Questionnaire (and CAGE-AID)

A quick, evidence-based tool for identifying potential alcohol or drug concerns. Simple and effective for raising awareness among employees.
● Purpose: A brief tool to identify potential alcohol misuse.● Format: 4 yes/no questions.● Use: Often given as a quick self-assessment; the CAGE-AID version includes drugs.● Scoring: 2+ “yes” answers suggest the need for further evaluation.● Source: Mayfield D, McLeod G, Hall P. (1974). The CAGE Questionnaire: Validation of a new alcoholism screening instrument. Am J Psychiatry, 131(10):1121-3.

DAST (Drug Abuse Screening Test)

Widely used to identify possible drug misuse, the DAST helps HR professionals promote early intervention and provide employees with the right resources.
● Purpose: Developed to screen for drug use problems (not including alcohol or tobacco).● Format: Available in 28-question or short 10-question versions.● Use: Provides insight into the extent of drug-related issues and possible need for referral.● Scoring: Higher scores indicate greater concern.● Source: Skinner HA. (1982). The Drug Abuse Screening Test. Addictive Behaviors, 7(4):363-371.

MAST (Michigan Alcohol Screening Test)

A trusted tool for detecting patterns of long-term alcohol use, supporting HR teams in guiding employees toward professional evaluation.
● Purpose: One of the oldest and most widely used screening tools for alcohol problems.
● Format: 22 yes/no questions, with shorter versions available (SMAST = 13 items, BMAST = 10 items).● Use: Helpful for identifying long-term patterns of alcohol misuse.● Scoring: Scores ≥6 suggest possible alcohol dependence.● Source: Selzer ML. (1971). The Michigan Alcoholism Screening Test: The quest for a new diagnostic instrument. Am J Psychiatry, 127(12):1653-1658.

How EAP Professionals Can Use These Tools

Screening resources empower EAP departments to foster employee wellness programs, encourage professional help-seeking, and reduce stigma in the workplace.
