  • Screening Questionnaires for Substance Use Awareness

Note for HR Professionals: These questionnaires are screening tools only. They cannot diagnose substance use disorders, but they can highlight potential concerns and encourage employees to seek professional help. HR staff may share them as confidential self-checks or through an Employee Assistance Program (EAP).

CAGE Questionnaire (and CAGE-AID)

 A quick, evidence-based tool for identifying potential alcohol or drug concerns. Simple and effective for raising awareness among employees.
● Purpose: A brief tool to identify potential alcohol misuse.● Format: 4 yes/no questions.● Use: Often given as a quick self-assessment; the CAGE-AID version includes drugs.● Scoring: 2+ “yes” answers suggest the need for further evaluation.● Source: Mayfield D, McLeod G, Hall P. (1974). The CAGE Questionnaire: Validation of a new alcoholism screening instrument. Am J Psychiatry, 131(10):1121-3.

DAST (Drug Abuse Screening Test)

Widely used to identify possible drug misuse, the DAST helps HR professionals promote early intervention and provide employees with the right resources.
● Purpose: Developed to screen for drug use problems (not including alcohol or tobacco).● Format: Available in 28-question or short 10-question versions.● Use: Provides insight into the extent of drug-related issues and possible need for referral.● Scoring: Higher scores indicate greater concern.● Source: Skinner HA. MAST (Michigan Alcohol Screening Test)

A trusted tool for detecting patterns of long-term alcohol use, supporting HR teams in guiding employees toward professional evaluation.
● Purpose: One of the oldest and most widely used screening tools for alcohol problems.
● Format: 22 yes/no questions, with shorter versions available (SMAST = 13 items, BMAST = 10 items).● Use: Helpful for identifying long-term patterns of alcohol misuse.● Scoring: Scores ≥6 suggest possible alcohol dependence.● Source: Selzer ML.
